Request for Waiver of Overpayments Resulting from Insufficient Deductions for Insurance Premiums

An employee appealed the denial of his application for waiver of an overpayment which resulted from insufficient payroll deductions for an optional life insurance plan. The request for waiver was denied since the employee continued to be covered by insurance throughout the period that premiums were not properly deducted, and the employee's failure to examine his Leave and Earnings Statements did not relieve him of responsibility.
