Request for Reconsideration of Denial of Waiver of Overpayment

An employee appealed a previous decision denying his request for waiver of indebtedness resulting from the agency's failure to deduct premiums for optional life insurance requested by the employee. Waiver may not be granted in spite of the fact that there had been a simultaneous increase in the basic premium for regular life insurance since the optional insurance premium was much greater, and the discrepancy should have been questioned by the employee.
