We sent letters to the heads of key departments and agencies, urging them to continue focusing on these issues. Below you can search only priority recommendations, or search all recommendations.
Our recommendations help congressional and agency leaders prepare for appropriations and oversight activities, as well as help improve government operations. Moreover, when implemented, some of our priority recommendations can save large amounts of money, help Congress make decisions on major issues, and substantially improve or transform major government programs or agencies, among other benefits.

Comments: DOD partially concurred with this recommendation. While DOD agreed with the intent of this recommendation, DOD stated that the MDA Director will determine which major integrated capabilities should be delivered via the Technical Capability Declaration (TCD) process. MDA also noted that it is updating MDA Directive 5000.17 (estimated release in 4Q FY2020), which is intended to address this recommendation. GAO will assess the revisions when they are available. We continue to believe that in order for the agency to meet the full intent of our recommendation, it should establish in policy, a clear, definitive standard for which capabilities require a TCD delivery.

Comments: DOD concurred with this recommendation and contends that its new Policy Memorandum 90 meets the intent of our recommendation. However, this new Policy Memorandum leaves open the possibility of continued inconsistent application of the Technical Capability Declaration (TCD) process. In order for the agency to meet the full intent of our recommendation, MDA should establish in policy, a clear, definitive standard for which capabilities require a TCD for delivery that should be tracked over several years to ensure that the policy is followed. MDA officials noted that revisions to MDA Directive 5000.17 expected at the end of Fiscal Year 2020 will address the recommendation. We will assess the new parameters set out in the revised policy directive, when it is available. We will also continue to track how MDA implements any new policies on TCDs and whether the information is transparent.

Comments: DOD concurred with this recommendation and has taken steps to implement it. While we noted in our report that the BMDS Accountability Report for 2018, approved by the MDA Director on March 9, 2018 reflected that the agency had taken steps to implement this recommendation, we plan to leave this recommendation open to ensure that the BMDS Accountability Report accurately contains all undefinitized contract actions that the agency enters into through the fiscal year and whether the reporting contains the information necessary to satisfy our recommendation.

Comments: DOD concurred with this recommendation and stated that MDA is actively working with the BMDS Operational Test Agency (BMDS OTA) to resolve any issues associated with, and the reporting of, modeling limitations. However, according to BMDS OTA officials, while the number of accredited models has risen in recent years, some remain unaccredited. We, therefore, believe it is premature to closeout this recommendation but we will continue to track MDA's progress on taking the necessary steps to implement this recommendation.

Comments: In July 2018, DOT released an initial plan related to this recommendation in response to congressional direction. This plan outlines DOT's overall approach for managing policy and research issues related to automated vehicles across DOT's modal administrations. In January 2020, DOT and the National Science and Technology Council released Ensuring American Leadership in Automated Vehicle Technologies (AV 4.0), building on prior policies that DOT has identified as actions regarding its implementation of GAO's recommendation. DOT has yet to identify, for example, performance measures to monitor and gauge results. Without a comprehensive plan, it continues to be unclear whether DOT is adequately tackling automated vehicle challenges.

Comments: FEMA concurred with this recommendation and said it would take steps to implement it. As of September 2018, officials reported completing activities to develop disaster-specific mitigation performance measures that align with strategic goals and analyzed available data to identify the drivers of mitigation in events of various sizes. Due to hiring delays associated with the establishment of the 406 Mitigation Branch, officials have extended the expected completion date for all actions, including proposing refined performance measures to FEMA senior leadership, to the end of January 2019. As of December 2019, GAO is awaiting a response from FEMA on their progress completing these actions.

Comments: FDA partially agreed with our recommendation. According to FDA, the agency is working on a plan to explore the viability of reaching cooperative arrangements with foreign regulatory bodies concerning imported aqua-cultured seafood. In exploring such arrangements, FDA stated that it will seek to explore a means by which the agency can leverage foreign regulatory bodies' seafood safety programs to provide additional oversight for seafood destined for the United States. According to FDA, such arrangements would be negotiated depending on the country's specific situation. We will continue to monitor FDA's specific efforts to implement this recommendation.

Comments: As of December 2019, FSIS had not yet acted on this recommendation. According to FSIS officials, the agency made a request to the USDA Office of General Counsel for their opinion on the legality of the recommendation. FSIS is waiting for OGC's response to that request. FSIS maintains that the information submitted by foreign countries as part of the equivalence determination that outlines their chemical residue monitoring plans and the review by the FSIS equivalence staff to ensure these countries employ an equivalent level of public health protection as that of the US already addresses this recommendation. We will continue to monitor how FSIS addresses this recommendation.

Comments: FDA agreed with this recommendation. According to FDA officials, the agency shared its testing methods for two drugs with FSIS and as of as of April 2019, FSIS and FDA were using the same method for measuring and confirming these two unapproved drugs. In August 2020, FDA told GAO that the agencies convene quarterly to discuss emerging and ongoing research needs in laboratory method development and the establishment of drug residue limits in seafood. We commend FDA and FSIS for taking these steps to share information on testing methods. However, GAO found that the agencies continue to use different multi-residue testing methods that look for different numbers of drugs--99 for FSIS and 40 for FDA--which results in the agencies using different maximum residue levels for some drugs. FDA's method can detect drugs that FSIS's does not and can detect some drugs at lower levels. FSIS's multi-residue method can detect 59 more drugs than FDA's method. The agencies do not have any plans to work on a multi-residue method both agencies can use.

Comments: According to FSIS officials in May 2020, the agency coordinates with FDA and EPA to carry out the National Residue Program, which entails testing FSIS-regulated products, including catfish, for chemical compounds of public health concern. FSIS officials indicated that FSIS will continue to use its own test methods that meet the agency's pre-defined quality assurance criteria, are applicable to the particular commodity under its jurisdiction, and fit its business model. Thus, FSIS currently does not have plans to work on a multi-residue method that both it and FDA can use on imported seafood, including catfish, as we have recommended.

They subsequently awarded a contract to assess the state of development for the GMM system and to provide recommendations on the actions CNCS needed to take in order to implement a commercial off-the-shelf (COTS) application for core grants management functions. According to CNCS officials, based on the findings from that assessment, further investments in developing customized applications (even an implementation of a COTS application) were not likely to be successful. As of September 2019, CNCS officials stated that they were pursuing the option of a federal shared service as a solution to grants management. Comments: Commerce agreed with this recommendation. In the Bureau's March 2017 action plan, it reported that as the Census Bureau develops enhancements to the procedures and training for enumerators, it will reflect on what it has learned from 2016 Census Test experiences as well as from recommendations such as this. It reported a target completion of December 2018, following completion and analyses of the nonresponse follow-up operation for the 2018 End-to-End Census Test. In January 2018, Bureau officials told us that leveraging enumerator collected information on the best time to conduct an interview would not be a part of questionnaire design's functionality for 2020 and will most likely rely on the system optimizer to determine the best time to contact a household. Bureau officials indicated they may push this recommendation out to 2030. During the 2018 Test, we continued to observe that enumerator notes were not being systematically reviewed by supervisors and managers, such that information such as office hours for apartment managers among other information we reported on was not being used by the operation. In June 2019, the Bureau informed us that it had added functionality to the enumerator device for enumerators to alert supervisors of case notes of "high importance" and was revising training to explain its use. Additionally, in December 2019, the Bureau shared documentation that reiterated the importance of enumerators reviewing prior case notes but clarified that enumerators should not expect their supervisors to regularly review those case notes. In April 2020, Bureau officials indicated that it was not likely that the Bureau would revise CFS training to systematically review case notes for the 2020 cycle, though we are following up with the Bureau to see if this can be done given the revised census time frames stemming from the COVID-19 outbreak. To fully implement this recommendation for 2020 and for future fieldwork, the Bureau needs to make better use of the information collected by enumerators during interview attempts about when to make additional attempts, such as during reported working hours of property managers for large multi-unit structures that house a large number of non-respondents.

Comments: The agency concurred with this recommendation. Since the issuance of GAO's report, in February 2016, Congress directed the Coast Guard to develop a long-term plan to cover fiscal year 2017 and 20 years thereafter and that it should be updated every two years. In November 2017, officials told GAO that the Coast Guard was developing a 20-year long-term plan that specifically focused on the highest priority recapitalization and sustainment efforts for its assets and will focus on meeting the intent of the 2016 congressional mandate. However, as of July 2020, the Coast Guard has not completed this plan. At that time, officials said that the Coast Guard continues to refine the process to define the long term acquisition and capital sustainment needs of the Service and align them with published and anticipated fiscal top line budgets. The Coast Guard is working with internal and external stakeholders to define useful parameters in order to complete work to close this recommendation. GAO will continue to monitor the Coast Guard's actions in completing its long-term plan given that GAO's recent work has found that the Coast Guard continues to pursue an unaffordable acquisition portfolio that is not likely to fully address all known and anticipated capability gaps.

Comments: DOD partially concurred with our 2013 recommendation that decisionmakers should have insight into the full lifecycle costs of MDA's weapon systems outlined in the Ballistic Missile Defense System Accountability Report (BAR), including the military services' operations and sustainment (O&S) costs. This is especially important, as after more than a decade MDA has yet to transfer weapon systems in production and sustainment to the military services, as originally intended. Consequently, MDA is becoming responsible for an increasing amount of the costs associated with these weapon systems. DOD and Congress have expressed concerns over this situation and are exploring a path forward; however, in the mean time, determining the O&S costs can help decisionmakers fully understand the financial responsibility for these weapon systems, be it with the military services or MDA. MDA cited beginning to report aspects of this information in the BAR and also establishing joint cost estimates (JCE) for O&S with the military services for some weapon systems, both of which could potentially serve as a means of providing decisionmakers with insight into the full lifecycle costs. We have an ongoing assessment that will evaluate both of MDA's cited efforts and the extent to which these are providing decisionmakers with a comprehensive understanding of the depth and breadth of each weapon system's full lifecycle costs.

Comments: DOD concurred with our 2013 recommendation regarding the need for MDA to stabilize its acquisition baselines, but also noted MDA's need to adjust its baselines to remain responsive to evolving requirements and threats; both of which are beyond MDA's control. Further, DOD highlighted the MDA Director's authority to make adjustments to the agency's programmatic baselines, within departmental guidelines. Our recommendation, however, is not designed to limit the Director's authority to adjust baselines or to prevent adjusting the baselines, as appropriate. Rather, our recommendation is designed to address traceability issues we have found with MDA's baselines, which are within its control. Specifically, for MDA to be able to effectively report longer-term progress of its acquisitions and provide the necessary transparency to Congress, it is critical that the agency stabilize its baselines so that once set, any revisions can be tracked over time. We have an ongoing assessment to update MDA's progress.

Comments: In March 2017, HUD stated that it has not developed a test to validate the performance of the whole-house ventilation specification. HUD reported that the Manufactured Housing Consensus Committee (MHCC) did not agree with the testing validation recommendations. As of March 2018, HUD continues to move forward in updating its standards, including a test for whole house ventilation. However, as part of the overall HUD effort to review regulations, HUD officials stated they are currently conducting a review of current and planned federal regulations for manufactured housing with the goal to facilitate the affordability of manufactured housing and to promote durable, safe, and cost effective construction techniques for manufactured homes. As a result, HUD does not anticipate moving forward with any manufactured housing regulations until it completes its review. We continue to believe that developing such a test will better ensure that air ventilation systems in manufactured homes perform as specified and meet the HUD Code and will continue to monitor HUD's progress in implementing our recommendation.

Comments: As of July 2020 there has been no change on the status of this recommendation. In June 2019, EPA reported to GAO that its tools for improving data about water systems compliance are not complete, though some states and laboratories have been exploring and testing incremental versions of them. EPA reported in a previous update that the agency had been working with states through face-to-face trainings and webinars on the reporting of milestone data. GAO will continue to monitor these efforts and reevaluate whether water systems' test results, corrective action milestones and violations are current, accurate and complete subsequent to the completion of the Compliance Monitoring Data Portal and the Safe Drinking Water Information System (SDWIS) Prime, described briefly below. However, until these new tools are complete, the status of this recommendation remains open. According to EPA, as of June 2018, SDWIS Prime has been available for exploring and testing, following incremental interim releases, since early 2018. In addition, EPA stated that SDWIS Prime will continue to be available for exploring and testing until the first production release anticipated for mid-2020.The agency has also focused on promoting electronic reporting of drinking water data through the development of the Compliance Monitoring Data Portal (CMDP). In June 2019, EPA stated that as of May 31, 2019, 10 states and more than 200 laboratories were using CMDP. According to EPA, as a result of using CMDP, these states reported a 30-70 percent reduction in staff time for data processing and a 90-99 percent reduction in errors for data.
