GAO’s recommendations database contains report recommendations that still need to be addressed. GAO’s priority recommendations are those that we believe warrant priority attention. We sent letters to the heads of key departments and agencies, urging them to continue focusing on these issues. Below you can search only priority recommendations, or search all recommendations.
Our recommendations help congressional and agency leaders prepare for appropriations and oversight activities, as well as help improve government operations. Moreover, when implemented, some of our priority recommendations can save large amounts of money, help Congress make decisions on major issues, and substantially improve or transform major government programs or agencies, among other benefits.

In its agency comment letter, the department noted that FEMA plans to engage field leaders on enhancements to its coach-and-evaluator program to develop a plan to address identified challenges. DHS also reported that FEMA plans to increase training offerings and align its curriculum so that FEMA Qualification System status matches workforce capability. DHS anticipates completion by March 31, 2021.

In its agency comment letter, the department stated that FEMA is working to finalize and implement some staff development initiatives we described in our report, such as the FEMA Deployment Performance Evaluation Directive, which will provide guidance on performance reviews for deployed staff. DHS anticipates that FEMA's efforts to implement will be completed by March 31, 2021.

Comments: In its February 2019 action plan, the Bureau indicated having updated the Census Field Supervisor hiring assessment to include questions on supervisory experience, in line with draft documentation provided near the end of our engagement. The Bureau also indicated that, by June 2019, it would communicate as part of supervisor training increased supervisory responsibilities and the need to more actively work with enumerators in answering casework questions. In August 2020, the Bureau informed us that the Bureau would not be altering the information flows for 2020 operations to ensure that census field supervisors receive the same guidance and procedural updates that managers within the area census office receive. To fully implement this recommendation for future fieldwork, the Bureau's planned or other actions will also need to demonstrate how the census field supervisors will have the information they need to carry out their responsibilities to provide supervisory support to enumerators.

Comments: In its February 2019 action plan, the Bureau indicated that it plans, by June 2019, to give area census offices (ACOs) the ability to distribute training and informational updates to their local workforces through the Operational Control System. As of January 2020, we are reviewing updated Bureau training documentation on planned mid-operation procedural changes. In April 2020, the Bureau informed us that ACOs would not be empowered during 2020 operations to deliver to their workforces standardized, mid-operation guidance that would be targeted to specific issues being observed locally in the field. Officials noted that they would revisit this issue after 2020 operations have concluded. To fully implement this recommendation for future fieldwork, the Bureau will need to demonstrate the ability of ACOs to identify procedural or other implementation issues encountered locally and develop some form of actionable guidance disseminated systematically to its workforce in near-real time.

Comments: VHA concurred with this recommendation and has provided regular updates on its progress in implementing it. As of April 2019, VHA's Office of Patient Advocacy (OPA) partnered with VA's Center for Healthcare Organization and Implementation Research (CHOIR) to better understand the current state of patient advocacy services in VHA, focusing on position descriptions, grade levels, and reporting structures. VA medical center staff completed questionnaires about the patient advocacy program in January 2019 and VHA analyzed the results. CHOIR officials are conducting site visits to interview key staff directly to identify the benefits and opportunities for improvements with patient advocacy services, including reporting structure. Upon completion of site visits to validate questionnaire findings, CHOIR will present their final recommendations to OPA. OPA will develop reporting structure guidance and work with workforce management and VHA senior leaders to communicate and implement the guidance. VHA's target completion for these efforts is December 2019.

Comments: VHA concurred with this recommendation and has provided regular updates on its progress in implementing it. As of April 2019, VHA's Office of Patient Advocacy (OPA) partnered with VA's Center for Healthcare Organization and Implementation Research (CHOIR) and VHA's Workforce Management to develop an evidence-based patient advocacy staffing model that accounts for facility size, complexity and geographic region. A set of questions was distributed to all VAMCs in December 2018. Responses to these questions have been analyzed by CHOIR, and on-site interviews at select facilities are in progress to validate the report findings. VHA's Workforce Management is working with CHOIR and OPA to use the results to develop a recommended and validated staffing model. This guidance will also be incorporated in the future revision of the VHA directive. The target completion of these efforts is December 2019.

Comments: VHA concurred with this recommendation and has provided regular updates on its progress in implementing it. As of April 2019, VHA's Office of Patient Advocacy (OPA) receives a weekly report from both the Patient Advocate Tracking System (PATS) and PATS-Replacement (PATS-R) Systems reporting on the number of new cases entered at every VA medical center (VAMC). With development of the PATS-R web-based tool, OPA, the Veterans Experience Office and the PATS-R developers have conducted a review of existing codes and are currently working with various VHA program offices to standardize codes across various data systems. VA plans to develop an auditing toolkit to ensure standardized, timely documentation of complaints, including accurate coding within PATS. The target completion date for these efforts is December 2019.

However, this joint assessment does not account for other aspects of the program, such as training, which was part of GAO's recommendation. According to Select Agent Program officials, the program is developing a new information system and officials plan to conduct a follow-up workload assessment once this new system is fully implemented, as they anticipate that the program will gain efficiencies once this new system is in place. Officials from the Select Agent Program said they did not have a definitive timeframe as to when the new system would be in place and a new workload assessment could be completed but, as of December 2019, they said it would be several years. Once the updated workforce assessment is completed, GAO will review it to determine if it fulfills the recommendation. Developing a joint workforce plan as recommended would help the program to better manage fragmentation by improving how it leverages resources, which in turn would help to ensure that all workforce and training needs are met.

Comments: The Department of Defense (DOD) concurred with this recommendation. In December 2018, the Marine Corps had completed some actions and has other ongoing actions intended to address the recommendation. For example, in June 2017 the Marine Corps issued the Marine Corps Ground Training Simulations Implementation Plan. The plan provides a framework for the Marine Corps' use of current and future simulations technology and virtual training environments to align training efforts and resource requirements. In addition, the Marine Corps continues to revise its training and readiness program manuals to articulate requirements that document training tasks, objectives, and required proficiency and reemphasize the importance of more effectively integrating ground simulations within current ground training approaches. Further, the Marine Corps is currently staffing a comprehensive Ground Simulations Training Reference Guide and is testing a new process, termed the Ground Simulation Training Effectiveness Process. This process will provide guidelines on conducting effectiveness analysis, including the selection of the devices to be evaluated and an identification of the data to be collected and assessed. As of August 2020, these actions remain in progress. Once fully implemented, these actions should help the Marine Corps more effectively and efficiently integrate virtual training devices into operational training, as GAO recommended in September 2017.

Comments: In May 2019, the agency provided an update of their recent efforts, including their revised Commercial Services Improvement Plan. This plan include performance goals and general time frames , however did not include performance measures or specific targets for each of the initiatives. The agency has subsequently reviewed this plan and determined that many of the goals and time frames identified were not realistic to achieve, making it difficult to establish performance measures or targets. As of July 2020, the agency is currently updating its 2015 Strategic Plan and reviewing the improvement plan to identify those items that will be feasible to implement. According to agency officials, they plan to develop goals and performance targets where feasible and develop time estimates for achieving them. Since this effort is currently underway, we will update the status of their efforts in the summer of 2021.

Comments: Commerce agreed with this recommendation. In the Bureau's March 2017 action plan, it reported that as the Census Bureau develops enhancements to the procedures and training for enumerators, it will reflect on what it has learned from 2016 Census Test experiences as well as from recommendations such as this. It reported a target completion of December 2018, following completion and analyses of the nonresponse follow-up operation for the 2018 End-to-End Census Test. In January 2018, Bureau officials told us that leveraging enumerator collected information on the best time to conduct an interview would not be a part of questionnaire design's functionality for 2020 and will most likely rely on the system optimizer to determine the best time to contact a household. Bureau officials indicated they may push this recommendation out to 2030. During the 2018 Test, we continued to observe that enumerator notes were not being systematically reviewed by supervisors and managers, such that information such as office hours for apartment managers among other information we reported on was not being used by the operation. In June 2019, the Bureau informed us that it had added functionality to the enumerator device for enumerators to alert supervisors of case notes of "high importance" and was revising training to explain its use. Additionally, in December 2019, the Bureau shared documentation that reiterated the importance of enumerators reviewing prior case notes but clarified that enumerators should not expect their supervisors to regularly review those case notes. In April 2020, Bureau officials indicated that it was not likely that the Bureau would revise CFS training to systematically review case notes for the 2020 cycle, though we are following up with the Bureau to see if this can be done given the revised census time frames stemming from the COVID-19 outbreak. To fully implement this recommendation for 2020 and for future fieldwork, the Bureau needs to make better use of the information collected by enumerators during interview attempts about when to make additional attempts, such as during reported working hours of property managers for large multi-unit structures that house a large number of non-respondents.

Comments: DOE stated that it will address our recommendation as part of its effort to meet the requirements of the Program Management Improvement Accountability Act of 2016 (Act). This Act requires the Office of Management and Budget (OMB) to adopt government-wide standards, policies, and guidelines for program and project management for agencies. In June 2018, OMB issued guidelines requiring federal agencies to submit a 5-year plan for implementing the Act. In November 2018, DOE submitted a draft implementation plan to OMB that described DOE's strategy for developing a DOE program management policy. In particular, its draft plan stated that DOE has established a working group to help develop the department's program management policy. To address our recommendation, DOE needs to finalize and issue its program management policy. We will continue to monitor DOE's actions.

Comments: DOE stated that it will address our recommendation as part of its effort to meet the requirements of the Program Management Improvement Accountability Act of 2016 (Act). This Act requires the Office of Personnel Management (OPM) to issue regulations identifying key program management skills and competencies, as well as to establish a new career path for program managers within an agency. In April and May of 2019, OPM issued guidance documents that identify competencies for program managers and update the job series classification for program managers. OPM officials also said that they have drafted a career path for program managers, which will highlight training and skills needed to progress in a program management career. In addition, DOE stated that it is holding working group meetings to develop strategies for program management training and certification. To address our recommendation, DOE needs to continue working with OPM and finalize its training program for program managers.

Comments: DOD did not concur with our recommendation to develop and implement performance measures for its credentialing program. In its response to the recommendation, DOD stated that servicemembers are not required to earn credentials and more than half of the credentials earned by servicemembers are voluntary. Therefore, establishing criteria that might create an incentive to force servicemembers into earning voluntary credentials would be counterproductive. DOD also stated that a basic reporting system is in place that captures credential attainment and associated costs that provides basic information to gauge the program's performance. As of April 2020, the department still does not plan to develop performance measures for the program.

Comments: DOD non-concurred with the recommendation. The Air Force has taken steps to address this recommendation. Specifically, Air Force officials stated they have completed one study and have an ongoing study, intended to reassess the assumptions underlying its annual training requirements for fighter aircrews. For example, Air Force officials stated a study was completed in August 2017 reassessing the criteria for designating aircrews as experienced or inexperienced for 4th generation fighter aircraft. In addition, Air Force officials stated that another study was intended to define the optimum mix of annual training requirements for fighter aircrews. These officials stated that the study results were provided to Air Force senior leaders in July 2018 for approval. As of August 2020, the Air Force did not provide any additional documentation on steps taken to address the recommendation. Completion of these studies and the corresponding adjustments to annual training requirements should help the Air Force ensure that their training plans are aligned to achieve a range of missions for current and emerging threats as recommended by GAO in September 2016.

Comments: The Department of Defense (DOD) concurred with this recommendation. As of December 2019, the Army had taken some steps to improve its guidance, as GAO recommended in August 2016, but did not plan to fully address the recommendation until 2021. Officials stated that the Army established target usage rates for existing virtual training devices and issued guidance and tracking tools for recording device usage. However, the Army had not modified the guidance, cited in GAO's August 2016 report, to require that training developers consider the amount of time available to train with or expected usage rates of new virtual training devices. According to Army officials, they will implement GAO's recommendation in a planned update to guidance on the justification and validation of new virtual training devices scheduled for 2021. By updating this guidance, the Army will have the information it requires to evaluate the amount of virtual training capabilities needed to achieve training tasks and proficiency goals during operational training.

Since May 2016, the Commanding General of Forces Command (FORSCOM) has chaired a Monthly Aviation Readiness Review (MARR) in which review members assess aviation readiness across all aviation organizations including UAS. In August 2018, Army Headquarters officials stated that the Army plans to update Army Regulation 220-1, Army Unit Status Reporting and Force Registration to "bring UAS Operator reporting in line with other Army weapon systems, as UAS readiness was not previously captured." However, as of November 2019, the revision to the Army Regulation had not been published.

Comments: DOD concurred with this recommendation. In October 2016, Army Headquarters officials stated that the Army had taken additional steps to mitigate potential risks posed by waiving course prerequisites for less experienced UAS pilots attending the course to become instructors. Specifically, by the end of fiscal year 2016, the Army had put 50 of 106 planned Universal Mission Simulators in place for active duty units and reduced the number of waivers granted for three of four course prerequisites. Army officials also provided documentation to show that the number of waivers granted had decreased in fiscal year 2016. However, an Army official from the Training and Doctrine Command stated that the Army had not provided additional training or preparation for instructors who had previously received a waiver of one of the course prerequisites to attend the instructor course as we had recommended. In July 2018, Army Headquarters officials indicated that the Army continued to use simulators to reduce the need for waivers but they also indicated that they continue to grant waivers to less experienced less experienced UAS pilots to enable them to enter the instructor operator course. In September 2019, the Army headquarters reported that the Army continues to use simulates to reduce the need for waivers to three of the four Army Instructor Operator (IO) course prerequisites (total hours, readiness level and aircraft currency). However as of November 2019 the Army had not provided additional training or preparation for instructors who had previously received a waiver of one of the course prerequisites to attend the instructor course as we had recommended.

Comments: DOD concurred with this recommendation and stated the Office of the Under Secretary of Defense for Personnel and Readiness was in the process of revising its draft "Department of Defense Training Strategy for Unmanned Aircraft Systems(UAS)" to address inter-service coordination to enable the department to train more efficiently and effectively as a whole. In October 2016, the Director stated that RAND had completed the draft strategy and that the Office of the Assistant Secretary of Defense (Readiness) had begun revising the strategy. An Office of the Assistant Secretary of Defense (Readiness) official working on the revisions stated that the strategy would address our recommendation and coordination among the services. However, as of October 2016, the Office of the Assistant Secretary of Defense (Readiness) had not yet issued the department-wide UAS training strategy. In May 2018, the Director Military Training and Ranges in the Office of the Assistant Secretary of Defense (Readiness)/Force Training engaged a team to review the 2016 draft strategy to recommend a way forward. According to that official, the review was nearing completion and he anticipated presenting their recommendation to the current Deputy Assistant Secretary of Defense for Force Education & Training in late July 2018. In September 2018 this official said that developing a new UAS strategy is not planned and he reiterated again in August 2019 that a UAS training strategy has not been issued. We continue to believe this is a valid recommendation and will keep it open in case the department eventually takes any relevant actions.

Comments: In July 2019, GAO confirmed that FAA still does not plan to collect General Aviation (GA) flight hour data during registration renewals or annual maintenance inspections because this would require rulemaking and could have a significant economic and paperwork impact on the GA industry. While FAA has made changes to the GA Activity Survey to improve the accuracy of the flight hour data collected for a sample of GA populations, FAA still does not plan to collect all GA flight hour data as part of its GA Activity Survey. GAO maintains that without comprehensive GA flight hour data, estimates from the GA Activity Survey may not be sufficient for drawing conclusions about changes in crash rates over time and that more precise flight hour data could allow FAA to better target its safety efforts within the GA industry.

Comments: In July 2019, GAO confirmed that FAA still does not plan to develop safety targets for different segments of the General Aviation (GA) industry. While FAA's General Aviation Joint Steering Committee was exploring metrics for monitoring different GA industry segments, it was determined that developing credible metrics was not feasible using the GA Activity Survey. GAO maintains that FAA needs to develop specific general aviation safety improvement targets for individual industry segments to support a data-driven, risk management approach.

However, as of July 2019 FAA has still not provided GAO with documentation of these performance measures. Comments: On July 19, 2017, OPM officials provided a document that summarized efforts that are underway to address the recommendation. According to the document, proposed changes/recommendations to training elements and to the SF-182 were presented to the Chief Learning Officers Council at their monthly meeting on June 19, 2017. According to OPM, its ongoing connection with the CLO Council will include gathering information through focus groups in the Fall of 2017 on agencies' talent development processes, tools, and procedures. Questions on how an agency prioritizes training and tools used to do so will be included. OPM says it will use information gathered to develop criteria for ranking training. OPM provided a sample training summary it prepares for each agency. The summary contains a section on utilization of learning by source type and utilization of learning by delivery method. These sections also contain data that can be used when comparing the merits of different delivery mechanisms and determining future use. On June 11, 2020, OPM told us that program officials are gathering support for its implementation of the recommendation and would soon provide that information to GAO.

Comments: On July 19, 2017 OPM provided a document that summarized efforts that are underway to address the recommendation. According to the document, OPM provided a refresher to the Chief Learning Officers Council on OPM's guidance on Reporting of Training Data. This included reviewing OPM's and agencies' responsibilities for the collection and reporting of data and reviewing the process of how agency data is transmitted to EHRI. Further, OPM says it highlighted the Guide to Human Resources Reporting as the source for the required format for reporting training data to EHRI and provided the Guide for Collection and Management of Training Information. Both guides are also located on OPM's website. OPM's training data report summaries encourage agencies to review their data, to check for submission errors and consistencies in reporting, and to validate that all training events are included. OPM reported that 3 CHCO agencies that did not submit any data for FY 14 submitted data for FY 15 and that EHRI is assisting agencies with data quality transmission and PII issues. On June 11, 2020, OPM told us that program officials are gathering support for its implementation of the recommendation and would soon provide that information to GAO.

Comments: FHWA has taken initial steps to implement a mechanism for state oversight, which includes collecting program implementation information from each state. FHWA plans to convene a working group to review this information and provide feedback to states on their programs. FHWA officials then anticipate sharing best practices and essential requirements for the program through webinars and other technical assistance. As of August 2019, FHWA has gathered information from states and is in the early stages of implementing this oversight mechanism.
