Recommendation: To help lessen confusion among the public and policy makers regarding federal data on sexual violence, the Director of OMB should establish a federal interagency forum on sexual violence statistics. The forum should consider the broad range of differences across the data collection efforts to assess which differences enhance or hinder the overall understanding of sexual violence in the United States.

Comments: At the time of our report, OMB neither agreed nor disagreed with this recommendation but stated that it did not believe convening a forum was the most strategic use of resources because agencies were not far enough along with their data collection efforts. We disagree with OMB's assertion because 7 of the 10 data collection efforts have been in place for more than 10 years, and several have been in place for multiple decades. As of December 2019, OMB has not provided information on any new efforts to establish a federal interagency forum on sexual violence statistics.

Recommendation: To make the best use of federal resources directed toward combating FGM/C in the United States, the Attorney General and the Secretaries of Education, Health and Human Services, Homeland Security, and State should each develop a written plan that describes the agency's approach for conducting education and outreach to key stakeholders in the United States regarding FGM/C.

Comments: The Department of Education (Education) concurred with this recommendation. In February 2020, Education told us that it had awarded three grants that, while not focused on FGM/C, may be used for student safety and health at the U.S. state and local levels. After it evaluates grantees' need for information and resources related to FGM/C, Education will determine its next steps. In addition, Education reported that it continues to participate on the federal interagency workgroup devoted to FGM/C and has dedicated program staff to respond to issues related to FGM/C. Education expects to finalize its written plan by August 2020.

Recommendation: To make the best use of federal resources directed toward combating FGM/C in the United States, the Attorney General and the Secretaries of Education, Health and Human Services, Homeland Security, and State should each communicate the plan with other relevant federal agencies and stakeholder groups, as appropriate.

Recommendation: To improve the effectiveness of DOD's strategy for preventing sexual assault in the military, as part of the department's next biennial update to the 2014-16 sexual-assault prevention strategy, the Secretary of Defense should direct the Under Secretary of Defense for Personnel and Readiness, in conjunction with the Secretaries of the military departments, to identify risk and protective factors for all of its domains, including the military community and its leaders.

Comments: DOD concurred with this recommendation. In April 2019, DOD issued a Prevention Plan of Action (PPoA) that will serve as a framework for preventing sexual assault. The PPoA contains 29 actions DOD will take to implement the prevention strategy. In March 2020, DOD officials stated that they had chartered a Prevention Collaboration Forum, which consists of subject matter experts, to address destructive behaviors which may share the same risk and protective factors as sexual assault. Additionally, DOD officials stated that research had begun on identifying the department's risk and protective factors. The officials expected the completed risk studies to be published internally in April 2020 and June 2020. We will continue to monitor DOD's efforts and update the recommendation's status when more information becomes available.

Recommendation: To help improve DOD's ability to measure the effectiveness of the department's efforts in preventing sexual assault in the military, the Secretary of Defense should direct the Under Secretary of Defense for Personnel and Readiness, in collaboration with the Secretaries of the military departments, to fully develop the department's performance measures for the prevention of sexual assault so that the measures include all key attributes of successful performance measures.

The PPoA contains 29 actions DOD plans to take to implement the prevention strategy, and instructs DOD to continuously evaluate sexual assault prevention activities. In December 2019, DOD officials stated that they were in the process of conducting an assessment of each of the services' efforts to implement the prevention strategy. Additionally, DOD officials stated that they are developing a milestone report to be issued by the end of fiscal year 2020 that will include updates on all of the department's efforts to prevent sexual assault. DOD is also planning to issue a report in fiscal year 2023 that will include a complete evaluation of the department's efforts to prevent sexual assault. We will continue to monitor DOD's efforts and update the recommendation's status when more information becomes available.

Recommendation: To help ensure widespread adoption and implementation of DOD's sexual-assault prevention strategy and to fulfill its role as a framework that can assist leaders and planners in the development of appropriate tasks, the Secretary of Defense should direct the Under Secretary of Defense for Personnel and Readiness, in conjunction with the Secretaries of the military departments, to ensure the military services' Sexual Assault Prevention and Response policies are aligned with the department's prevention strategy.

In that plan, DOD generally outlined its intent to develop metrics to assess prevention and response efforts pertaining to males who experience sexual assault. In July 2019, officials from DOD's Sexual Assault Prevention and Response Office stated that DOD's efforts outlined in the October 2016 plan include data-driven decision making. However, DOD has not provided evidence that it has developed a plan for data-driven decision making to prioritize its sexual assault prevention efforts. We will continue to monitor DOD's efforts to address this recommendation.

Recommendation: To improve DOD's ability to prevent sexual assaults of male servicemembers, to increase its responsiveness to male servicemembers who are sexually assaulted, and to address challenges faced by male servicemembers as DOD continues to seek to transform its culture to address sexual assault, the Secretary of Defense should direct the Under Secretary of Defense for Personnel and Readiness, in collaboration with the Secretaries of the military services, to develop clear goals with associated metrics to drive the changes needed to address sexual assaults of males and articulate these goals, for example in the department's next sexual assault prevention strategy.

Comments: DOD concurred with this recommendation in written comments on our report. In October 2016, DOD issued a plan to prevent and respond to sexual assault of military men. In August 2018, DOD's Sexual Assault Prevention and Response Office (SAPRO) stated that objectives 1-4 of this plan constitute DOD's goals to address sexual assaults of military men. However, the plan does not contain metrics; one of the plan's objectives is to develop metrics to assess prevention and response efforts pertaining to men who experience sexual assault. In July 2019, SAPRO officials stated that they are waiting to complete most actions for objectives 1-3 of the plan before developing associated metrics, and that completion of the metrics is expected by 2024. We will continue to monitor DOD's planned development of metrics for its prevention and response efforts for sexual assault of male servicemembers.

Recommendation: To improve DOD's ability to prevent sexual assaults of male servicemembers, to increase its responsiveness to male servicemembers who are sexually assaulted, and to address challenges faced by male servicemembers as DOD continues to seek to transform its culture to address sexual assault, the Secretary of Defense should direct the Under Secretary of Defense for Personnel and Readiness, in collaboration with the Secretaries of the military services, to include information about the sexual victimization of males in communications to servicemembers that are used to raise awareness of sexual assault and the department's efforts to prevent and respond to it.

DOD's October 2016 plan to prevent and respond to sexual assault of military men included objectives to develop a unified communications plan tailored to men across DOD, and to improve servicemember understanding of sexual assault against men. Recommendation: To improve DOD's ability to prevent sexual assaults of male servicemembers, to increase its responsiveness to male servicemembers who are sexually assaulted, and to address challenges faced by male servicemembers as DOD continues to seek to transform its culture to address sexual assault, the Secretary of Defense should direct the Under Secretary of Defense for Personnel and Readiness, in collaboration with the Secretaries of the military services, to revise sexual assault prevention and response training to more comprehensively and directly address the incidence of male servicemembers being sexually assaulted and how certain behavior and activities--like hazing--can constitute a sexual assault.

DOD's October 2016 plan to prevent and respond to sexual assault of military men contained objectives to develop research-informed training for servicemembers involved with sexual assault prevention and response to improve understanding of male victimization and how to prevent the crime, commander/supervisor training; and gender-responsive treatment guidelines for providers, among other things. Officials of DOD's Sexual Assault Prevention and Response Office stated in July 2019 that DOD expects to complete these tasks in 2021.

Recommendation: To improve DOD's ability to prevent sexual assaults of male servicemembers, to increase its responsiveness to male servicemembers who are sexually assaulted, and to help ensure that all of DOD's medical and mental health providers are generally aware of any gender-specific needs of sexual assault victims, and that victims are provided the care that most effectively meets those needs, the Assistant Secretary of Defense for Health Affairs should, in collaboration with the services' Surgeons General, develop and issue guidance for the department's medical and mental health providers--and other personnel, as appropriate--based on the results of this evaluation that delineates these gender-specific distinctions and the care regimen that is recommended to most effectively meet those needs.

In August 2018, the Office of the Assistant Secretary of Defense for Health Affairs stated that DOD is writing an Instruction and two accompanying Defense Health Agency Procedural Instructions that will delineate gender-specific distinctions and care regimens where appropriate. The guidance will incorporate the findings of a report by the Psychological Health Center of Excellence of the Defense Health Agency resulting from a planned study of the patterns of health care utilization of servicemembers reporting a sexual assault, including any gender differences. As of September 2019, DOD had not yet issued the guidance. We will continue to monitor DOD's actions to implement this recommendation.
