A retired Air Force member's former spouse claimed entitlement to her former husband's Survivor Benefit Plan (SBP) annuity, contending that her former husband properly filed his SBP election pursuant to a court order. GAO held that: (1) the Air Force improperly determined that the member's current spouse was the SBP beneficiary because the SBP election form lacked the claimant's signature; (2) the Air Force failed to officially notify the claimant that her required signature had not been provided on the election form; (3) there was no provision for providing dual SBP coverage; and (4) the Air Force should determine whether the invalid election resulted from an administrative error and take appropriate corrective action to validate the SBP election.
