The last question in the survey asks you to indicate if you have completed
this survey. By clicking on "complete", this tells us that your answers are official
and final. Please note that your answers will not be used unless you have clicked
"complete".
To save and exit: You may complete the questionnaire in one or multiple
sessions. To end a session,
or upon completion of the questionnaire, click on the "Save and Exit" button at
the bottom of the screen. Always use the
"Save and Exit" button to close the questionnaire. If you do not, you will
lose all the information that
you have entered on the screen of the questionnaire.
