    Recommendation: Given DOT's new discretionary grant programs and similar challenges we have found with previous DOT programs, the Secretary of Transportation should issue a directive that governs department-wide and modal administration discretionary grant programs. Such a directive should include requirements to: (1) develop a plan for evaluating project proposals in advance of issuing a notice of funding availability that defines the stages of the process, including how the process will be overseen to ensure a consistent review of applications; (2) document key decisions, including the reason for any rating changes and the officials responsible for those changes, and how high-level concerns raised during the process were addressed; and (3) align stated program purpose and policy priorities with the evaluation and selection process.

    Comments: DOT concurred with this recommendation. To fully implement this recommendation, DOT must issue a directive, to address our concerns and require DOT's operating administrations to develop and document procedures for all phases of the process for its discretionary grants.
    Recommendation: To better address FTA's stated policy priority of regional resilience and to minimize fragmentation of federal efforts, the Secretary of Transportation should direct the FTA Administrator to (a) examine the projects FTA funded under the Disaster Relief Appropriations Act of 2013 (DRAA) discretionary transit resilience grant program for potential duplication with other resilience efforts and (b) determine whether it is appropriate to realign funds for FTA-supported projects for other purposes authorized under the DRAA, or request a rescission of funds for any of the FTA-supported projects.

    Recommendation: As DOT, in response to the Fixing America's Surface Transportation (FAST) Act, conducts additional evaluation and analysis of ECP brakes and updates the regulatory impact analysis, the Secretary of Transportation should direct the Federal Railroad Administration (FRA) and Pipeline and Hazardous Materials Safety Administration (PHMSA) to take into account, in the updated regulatory impact analysis conducted in response to the FAST Act, potential uncertainty in key variables and assumptions, such as, but not limited to, fuel prices and future rail traffic of crude oil and ethanol, discuss this uncertainty, and present ranges of possible scenarios.

    Comments: DOT disagreed with this recommendation, asserting that we based our findings for this recommendation only on the views of industry and experts interviewed as part of our review. However, our justification for this recommendation is primarily based on data that DOT received from railroads, on what DOT reported in the rulemaking documents, and on clear criteria from OMB and others for conducting such analyses. To fully implement this recommendation, DOT should update its 2015 ECP Brake rule and the supporting regulatory impact analyses for ECP brakes, DOT should provide a range of possible scenarios and acknowledge uncertainty in certain estimates which could help increase confidence in those estimates and address stakeholder concerns.
    Recommendation: As DOT, in response to the FAST Act, conducts additional evaluation and analysis of ECP brakes and updates the regulatory impact analysis, the Secretary of Transportation should direct FRA and PHMSA to create a plan to collect data from railroads' ongoing and future operational experiences using ECP brakes. The plan should include details on how the agency will work with railroads to collect this data, ensure that such data are reliable, and analyze these data to conduct a retrospective analysis of the ECP brakes requirement that could help inform any potential future actions regarding ECP brakes.

    Recommendation: The Secretary of the Department of Transportation should direct the FHWA Administrator to develop an efficiency measure or measures that demonstrate the linkage between the federal funding of bridges and the desired performance outcomes, such as maintained or improved bridge conditions, and report the resulting information to Congress.

    Comments: In October 2016, the U.S. Department of Transportation's Federal Highway Administration (FHWA) reported to GAO that FHWA will review bridge project information and cost data that is currently being collected and review Office of Management and Budget's guidance and other examples for developing meaningful performance measures and measures that were proposed in prior FHWA proposed rulemaking efforts. Following those reviews, FHWA reported that an applicable efficiency measure will be developed that links federal funding and condition of bridges. FHWA reported that this review and evaluation will be completed by January 31, 2018 and once the efficiency measure is implemented it will be reported and incorporated in one of FHWA's existing reports to Congress.

    Recommendation: To improve access to and awareness and applicability of ITS resources for ITS deployment, the Secretary of Transportation should direct the ITS Joint Program Office (JPO), in coordination with the Federal Transit Administration (FTA), to develop a strategy to raise awareness of JPO's training, technical assistance, and knowledge resources for transit ITS deployment in the transit community.

    Comments: The Department of Transportation agreed with GAO's recommendation and stated that its Professional Capacity Building Program has two initiatives under development that will raise awareness of existing Intelligent Transportation System knowledge resources. First, the department will develop an overall course catalog that will describe all existing resources offerings. Second, the department will develop a new strategic plan that will utilize information from the updated course catalog as well as internal analyses to determine which new knowledge resources need to be developed to meet the needs of the transit community. As of June 2017, GAO is awaiting the Department's response regarding the status of its efforts to implement this recommendation.
    Recommendation: To improve access to and awareness and applicability of ITS resources for ITS deployment, the Secretary of Transportation should direct the ITS JPO, in coordination with FTA, to include ITS adoption by small urban and rural transit providers in ITS monitoring efforts.

    Recommendation: To promote the transition to improved crash test standards, to strengthen FHWA's oversight of the roadside safety hardware's crash-testing process, and to make more information available to states and industry on how roadside safety hardware performs in actual conditions, the Secretary of Transportation should direct the Administrator of FHWA to direct FHWA's division offices to help ensure, through their oversight of states' standards and design specifications, that states have written policies in place to require the installation of appropriately crash-tested roadside safety hardware on the National Highway System to address inconsistent practices across states.

    Comments: DOT concurred with this recommendation and stated that FHWA will review existing state policies and procedures regarding the installation of appropriately crash-tested roadside safety hardware on the National Highway System and complete this review by July 2017. Based on these findings, DOT stated that FHWA will, by December 2017, direct its division offices to work with state Departments of Transportation to modify or improve state DOT written policies as appropriate.
    Recommendation: To promote the transition to improved crash test standards, to strengthen FHWA's oversight of the roadside safety hardware's crash-testing process, and to make more information available to states and industry on how roadside safety hardware performs in actual conditions, the Secretary of Transportation should direct the Administrator of FHWA to monitor and periodically report to Congress (or report through the agency's publicly available website) progress states and the industry are making in transitioning to the MASH crash-testing standards for roadside safety hardware.

    Comments: DOT concurred with this recommendation and stated FHWA will collect information on states' progress in implementing a policy to use MASH-tested hardware and post this information on FHWA's website. DOT also stated that FHWA will coordinate with the American Association of State Highway and Transportation Officials to monitor the progress of the MASH transition and to have this monitoring process in place by July 2017.
    Recommendation: To promote the transition to improved crash test standards, to strengthen FHWA's oversight of the roadside safety hardware's crash-testing process, and to make more information available to states and industry on how roadside safety hardware performs in actual conditions, the Secretary of Transportation should direct the Administrator of FHWA to provide additional guidance to crash test labs and accreditation bodies to ensure that labs have a clear separation between device development and testing in cases where lab employees test devices that were developed within their parent organization.

    Comments: DOT concurred with this recommendation and stated that FHWA will conduct outreach to determine how conflicts of interest are viewed and addressed by labs and by the accrediting bodies. FHWA will also determine how to most effectively provide any additional guidance on mitigating potential these conflicts within its current legal authority. DOT stated FHWA will complete this outreach by June 2017 and issue additional guidance communicating FHWA's expectations by December 2017.
    Recommendation: To promote the transition to improved crash test standards, to strengthen FHWA's oversight of the roadside safety hardware's crash-testing process, and to make more information available to states and industry on how roadside safety hardware performs in actual conditions, the Secretary of Transportation should direct the Administrator of FHWA to develop a process for third-party verification of results from crash-test labs.

    Comments: DOT concurred with this recommendation and stated that FHWA will conduct outreach to identify potential processes for third party review of results from crash labs. DOT stated FHWA will complete this outreach by July 2017.
    Recommendation: To promote the transition to improved crash test standards, to strengthen FHWA's oversight of the roadside safety hardware's crash-testing process, and to make more information available to states and industry on how roadside safety hardware performs in actual conditions, the Secretary of Transportation should direct the Administrator of FHWA to support additional research and disseminate results on roadside safety hardware's in-service performance, either as part of future phases of FHWA's current pilot study on guardrail end terminals' performance or as part of FHWA's broader research portfolio.

    Recommendation: To better ensure that federal data center consolidation and optimization efforts improve governmental efficiency and achieve cost savings, the Secretaries of the Departments of the Interior, State, Transportation, and the Treasury; the Administrators of the Environmental Protection Agency, National Aeronautics and Space Administration and Small Business Administration; the Directors of the National Science Foundation and Office of Personnel Management; and the Chairman of the Nuclear Regulatory Commission should take action to address challenges in establishing, and to complete, planned data center cost savings and avoidance targets for fiscal years 2016 through 2018.

    Comments: The Department of Transportation (Transportation) agreed with our recommendation, and has taken initial steps to implement it. In April 2016, the department stated in correspondence to GAO that it's Office of the Chief Information Officer (OCIO) was actively engaging with the department's Operating Administrations and was reconciling its original cost savings and avoidance targets to develop and update a yearly calculation as part of Transportation's multi-year strategy to consolidate and optimize its data centers. The department added that periodic updates would be provided to OCIO leadership and the CIO Council, with reconciled cost savings and avoidance targets for fiscal years 2017 and 2018 expected to be updated by September 30, 2016. However, as of July 2017, Transportation has not updated its Data Center Optimization Strategic Plan to include planned cost savings and avoidances targets for fiscal years 2016 through 2018. We will continue to monitor and evaluate the department's progress in implementing this recommendation and update accordingly.
    Recommendation: To better ensure that federal data center consolidation and optimization efforts improve governmental efficiency and achieve cost savings, the Secretaries of the Departments of the Interior, State, Transportation, and the Treasury; the Administrators of the Environmental Protection Agency, National Aeronautics and Space Administration and Small Business Administration; the Directors of the National Science Foundation and Office of Personnel Management; and the Chairman of the Nuclear Regulatory Commission should take action to address challenges in establishing, and to complete, planned data center cost savings and avoidance targets for fiscal years 2016 through 2018.

    Comments: The Department of State agreed with our recommendation, and has taken initial steps to implement it. In June 2016, the department stated in correspondence to GAO that it was in the process of reviewing pending guidance on the Office of Management and Budget's Data Center Optimization Initiative (DCOI). FTA announced the selection of 19 Rides to Wellness Demonstration and Innovative Coordinated Access and Mobility pilot projects, totally about $7.3 million, in September 2016. According to FTA, seven of the 19 projects have a NEMT focus. In addition, during 2016 FTA conducted outreach sessions in various cities throughout the United States to discuss partnership opportunities between the health and transportation industries. FTA stated that a new series of NEMT-specific listening sessions is now underway and additional sessions are planned for 2017. The purpose of these sessions is to identify challenges related to NEMT coordination that will be addressed by Coordinating Council on Access and Mobility (CCAM) working groups in the future. CCAM is also in the process of developing a strategic plan framework. FTA anticipates agreement on this framework in December 2016. The Fixing America's Surface Transportation (FAST) Act requires CCAM to develop and publish a strategic plan. FTA anticipates issues related to this recommendation will be developed through 2020. We plan to keep this recommendation open to further monitor CCAM and FTA progress and the extent challenges are identified and addressed related to better coordinating Medicaid and VA NEMT with other NEMT programs in the federal government, through the strategic plan framework or other actions resulting from the listening sessions.

These proposed regulations partly address our recommendation by proposing new rules to address risks posed by new gathering pipeline construction, but do not fully address our recommendation to subject these pipelines to emergency response planning requirements, particularly for hazardous liquid pipelines. As of October 2016, PHMSA has determined that it needs one year's worth of data before it can begin assessing what regulations are needed to fully address the risks of hazardous liquid pipelines.

    Recommendation: In order to improve the management of DOD's headquarters-reduction efforts, the Secretary of Defense should reevaluate the decision to focus reductions on management headquarters to ensure the department's efforts ultimately result in meaningful savings.

    Comments: DOD partially concurred with our recommendation, stating that this department-wide recommendation would garner greater savings but did not provide plans on how it planned to implement the recommendation. As of March 2017, DOD was more broadly defining management headquarters and applying headquarters reductions to a wider range of organizations, but the department has not fully taken steps to re-evaluate the decision to focus reductions on management headquarters, as GAO recommended in June 2014. In an August 2015 memorandum, the Deputy Secretary of Defense noted that a comprehensive definition of major headquarters activities had been established, and he directed DOD to update the department's guiding instruction on management headquarters and databases in an effort to more broadly account for headquarters resources. The memorandum also directed a 25 percent reduction across all appropriations funding from fiscal years 2017 through 2020 for these headquarters activities in lieu of the 20 percent requirement previously established by the department. As of September 2016, a DOD official stated the department had not completed efforts to rebaseline all of its components according to the comprehensive definition of major headquarters activities, which is needed to determine what elements of the components are considered headquarters so the department can apply relevant reductions to its budget submission. In addition, Section 346 of the National Defense Authorization Act for Fiscal Year 2016 requires that the Secretary of Defense implement a plan to ensure the department achieves not less than $10 billion in cost savings from the headquarters, administrative, and support activities of the department by fiscal year 2019. The legislation also directed the Secretary of Defense to modify DOD's headquarters reduction plans to ensure that it achieves savings in total funding for major headquarters activities of not less than 25 percent of the baseline amount in fiscal year 2016 by fiscal year 2020. In a March 2016 letter to the Armed Services Committee, DOD stated that it is focusing on broad efficiency initiatives beyond reductions in management headquarters. While DOD has taken some steps to achieve greater savings by applying additional headquarters reductions to more organizations, an official stated DOD will not document these actions until it submits its budget request for fiscal year 2018 in the spring of 2017. Until DOD documents the reductions based on its broader efficiency initiatives, GAO cannot determine if DOD has fully implemented this action. We will continue to monitor actions DOD takes in response to this recommendation and will provide updated information as appropriate.

    Comments: The Department of Defense concurred with our recommendation to set clearly defined and consistently applied starting point as a baseline for the reductions. As of March 2017, DOD had taken some steps to set a clearly defined and consistently applied starting point as a baseline for headquarters reductions, but its efforts are not yet complete. In its response to GAO's recommendation, DOD recommended the use of the Future Years Defense Program data to set the baseline going forward. It stated that it was enhancing the data elements within DOD's Resource Data Warehouse to better identify management headquarters resources to facilitate tracking and reporting across the department. A December 2014 Resource Management Decision directed DOD components to identify and correct inconsistencies in major headquarters activities in authoritative DOD systems, and reflect those changes in the fiscal year 2017 program objective memorandums or submit them into the manpower management system. As of November 2016, a DOD official stated that the department had taken steps to use this definition as a baseline for headquarters reductions. However, according to this official, the department will not identify the baseline until it submits its budget request for fiscal year 2018 in the spring of 2017. Until DOD completes its efforts, savings to management headquarters will likely be difficult to track, and the department may not be assured that the reductions are achieved as intended. We will continue to monitor actions DOD takes in response to this recommendation and will provide updated information as appropriate.

In its response to GAO's June 2014 report, DOD noted that by using the Future Years Defense Program data to set the baseline, it would be able to track and report changes to Congress. DOD further stated that it was enhancing data elements within DOD's Resource Data Warehouse to better identify management headquarters resources across the department. GAO agreed that these enhancements to data elements would increase DOD's capability to track and report management headquarters across the department, and thus, the Future Years Defense Program could be used to set baselines and track future reductions.     Recommendation: The Director of the Office of Management and Budget should direct the Federal Chief Information Officer to update, and clearly and explicitly issue incremental development guidance that addresses the following three components: (1) requires projects associated with major IT investments to deliver incremental functionality at least every 12 months, with the exception of the three types of investments identified in this report; (2) specifies how agencies are to define the project functionality that is to be delivered; and (3) requires agencies to define a process for enforcing compliance with incremental functionality delivery, such as the use of TechStat sessions.

    Comments: The Office of Management and Budget (OMB) agreed with our recommendation to update and issue incremental guidance, but disagreed that the time frame for incremental delivery should be changed to every 12 months. Subsequently, OMB began to take steps to address aspects of our recommendation. Specifically, in June 2014, OMB updated its information technology (IT) budget guidance to, among other things, define project functionality as any changes to an IT system that primarily provides new or improved capability to the end user. Additionally, in June 2015, OMB issued its guidance on how agencies are to implement December 2014 federal IT acquisition reform legislation. As part of that guidance, OMB required CIOs to ensure that all acquisition strategies and acquisition plans that include IT apply adequate incremental development principles. However, as of June 2017, OMB's annually updated IT budget capital planning guidance still requires that projects associated with major IT investments deliver functionality every 6 months, rather than every 12 months, as we recommended. In the absence of our recommended delivery time frame change, OMB is at risk of continuing to require functionality to be delivered in a time frame that we found to be unrealistic for many IT investments based on their current levels of performance. We will continue to evaluate OMB's progress in implementing the recently issued guidance and in considering a change in how often projects are to deliver functionality.
    Recommendation: The Secretaries of Defense, Health and Human Services, Homeland Security, and Transportation should modify, finalize, and implement their agencies' policies governing incremental development to ensure that those policies comply with OMB's guidance, once that guidance is made available.

    Recommendation: To improve the CSA program, the Secretary of Transportation should direct the FMCSA Administrator to revise the SMS methodology to better account for limitations in drawing comparisons of safety performance information across carriers; in doing so, the Secretary of Transportation should direct the FMCSA Administrator to conduct a formal analysis that specifically identifies: (1) limitations in the data used to calculate SMS scores including variability in the carrier population and the quality and quantity of data available for carrier safety performance assessments, and (2) limitations in the resulting SMS scores including their precision, confidence, and reliability for the purposes for which they are used.

    Comments: As of October 2016, FMCSA continues to maintain that they do not agree with our methodology or conclusions. While FMCSA's position about our specific recommendation is unchanged, FMCSA noted that Section 5221 of the FAST Act directed the National Academies of Science (NAS) to conduct a safety correlation study of the CSA program, and specifically FMCSA's Safety Measurement System's (SMS) methodology. FMCSA stated that if the outcome of the NAS study results in recommendations for SMS changes, they will address those recommendations accordingly. We continue to believe this recommendation has merit and could help the agency better target FMCSA's resources to the carriers that pose the highest risk of crashing, as we demonstrate in our report. For example, we reported that FMCSA requires a minimum level of information for a carrier to receive an SMS score; however, this requirement is not strong enough to produce sufficiently reliable scores. As a result, GAO found that FMCSA identified many carriers as high risk that were not later involved in a crash, potentially causing FMCSA to miss opportunities to intervene with carriers that were involved in crashes. FMCSA's methodology is limited because of insufficient information, which reduces the precision of SMS scores. GAO found that by scoring only carriers with more information, FMCSA could better identify high risk carriers likely to be involved in crashes. This illustrative approach involves trade-offs; it would assign SMS scores to fewer carriers, but these scores would generally be more reliable and thus more useful in targeting FMCSA's scarce resources.

    Recommendation: To ensure that the increasing risks of GPS disruptions to the nation's critical infrastructure are effectively managed, the Secretary of Homeland Security should increase the reliability and usefulness of the GPS risk assessment by developing a plan and time frame to collect relevant threat, vulnerability, and consequence data for the various critical infrastructure sectors, and periodically review the readiness of data to conduct a more data-driven risk assessment while ensuring that DHS's assessment approach is more consistent with the National Infrastructure Protection Plan (NIPP).

    Comments: DHS officials had previously indicated that DHS's Office of Infrastructure Protection (IP) and Office of Cyber and Infrastructure Analysis (OCIA) have discussed an update of the GPS risk assessment, noting that such an update may be included in fiscal year 2017 planning documents. However, as of February 2017, no documentation had been provided that demonstrates such plans. Additionally, information from DHS shows that DHS has continued other efforts to collect potentially relevant threat, vulnerability, and consequence data for various GPS equipment in use. For example, according to DHS officials, DHS has conducted visits to major maritime, finance, wireless communications, and electricity firms to gauge their understanding of GPS vulnerabilities and of technology- and strategy-based efforts to improve GPS resilience, and DHS documentation shows that DHS has held events to test GPS receivers as part of assessing vulnerabilities. We will update the status of this recommendation after we receive additional information from DHS.
    Recommendation: To ensure that the increasing risks of GPS disruptions to the nation's critical infrastructure are effectively managed, the Secretary of Homeland Security should, as part of current critical infrastructure protection planning with Sector-Specific Agencys (SSAs) and sector partners, develop and issue a plan and metrics to measure the effectiveness of GPS risk mitigation efforts on critical infrastructure resiliency.

    Comments: As of February 2017, DHS documentation shows that DHS has worked with Sector Specific Agencies (SSAs) and other interagency partners to help manage GPS risks and continues to communicate information on risks to critical infrastructure partners. For example, according to DHS officials, this included briefing field staff and developing questions for infrastructure surveys to gather information on GPS resilience at the facility level. According to DHS officials, at the national level DHS included GPS in discussions with SSAs on topics they could include in their Sector-Specific Plans (each SSA develops a Sector-Specific Plan to detail risk management in its critical infrastructure sector), but DHS has also indicated that sector-oriented metrics are not a viable means of assessing risk management actions. We will update the status of this recommendation after we receive additional information from DHS.
    Recommendation: To improve collaboration and address uncertainties in fulfilling the National Security Presidential Directive 39 (NSPD-39) backup-capabilities requirement, the Secretaries of Transportation and Homeland Security should establish a formal, written agreement that details how the agencies plan to address their shared responsibility. This agreement should address uncertainties, including clarifying and defining DOT's and DHS's respective roles, responsibilities, and authorities; establishing clear, agreed-upon outcomes; establishing how the agencies will monitor and report on progress toward those outcomes; and setting forth the agencies' plans for examining relevant issues, such as the roles of SSAs and industry, how NSPD-39 fits into the NIPP risk management framework, whether an update to the NSPD-39 is needed, or other issues as deemed necessary by the agencies.

    Recommendation: To improve the usefulness of southwest border crossing wait time data for informing public and management decisions, the Commissioner of CBP should, in consultation with Federal Highway Administration and state DOTs, assess the feasibility of replacing current methods of manually calculating wait times with automated methods, which could include assessing all of the associated costs and benefits, options for how the agency will use and publicly report the results of automated data collection, the potential trade-offs associated with moving to this new system, and other factors such as those influencing the possible expansion of existing automation efforts to the 34 other locations that currently report wait times but have no automation projects under way.

    Comments: As of May 2017, CBP's Office of Field Operations (OFO) reports working to identify a feasible and cost effective wait time solution to measure commercial vehicle delays along the southern border. Specifically, CBP officials report that they have been partnering with the Federal Highway Administration and the Texas A&M's Transportation Institute on the deployment of an automated radio-frequency identification measurement solution to measure commercial delays at eight crossings. To verify the accuracy of the automated wait time data, CBP officials report that in June 2016 they conducted a ground-truth analysis with mixed results. CBP officials report DHS Science and Technology directorate delivered their final report in February 2017 and by the end of September 2017, pending review and acceptance of the report's findings, CBP will coordinate efforts to develop the required communication protocols and data schematics for near real-time commercial vehicle wait time updates to the CBP Border Wait Time website and Border Wait Time app. CBP estimates that this recommendation will be completed in October 2017.
    Recommendation: To better ensure that CBP's Office of Field Operations' (OFO) staffing processes are transparent and to help ensure CBP can demonstrate that these resource decisions have effectively addressed CBP's mission needs, the Commissioner of CBP should document the methodology and process OFO uses to allocate staff to land ports of entry on the southwest border, including the rationales and factors considered in making these decisions.
