    Recommendation: To better ensure grantees' compliance with the Drug-Free Communities Support Program's statutory requirements and to strengthen monitoring of grantee activities, SAMHSA should develop an action plan with time frames for addressing any deficiencies it finds through its reviews and making systemic changes to mitigate deficiencies on a prospective basis to strengthen the grant monitoring process.

    Comments: As of April 2017, according to the Department of Health and Human Services, SAMHSA is currently in the process of implementing routine audits of all its grant files. SAMHSA will also conduct an audit specifically of DFC files to ensure compliance by the end of summer 2017. Additionally, SAMHSA will offer regular training to government project officers (GPO) on grant report requirements and will ensure DFC GPO participation. Finally, SAMHSA is actively implementing the Grants Enterprise Management System to streamline the grants management process and make the process for maintaining grant files automated. By the end of fall 2017, all newly awarded DFC grants will be in this system. By the end of fall 2018, all active DFC grants will utilize this system.
    Recommendation: To better ensure grantees' compliance with the Drug-Free Communities Support Program's statutory requirements and to strengthen monitoring of grantee activities, SAMHSA should develop and implement a method for ensuring that the grantee status reports it provides to ONDCP are complete and accurate.

    Recommendation: To improve VA's supportive-housing EUL program and meet the needs of homeless veterans, the Secretary of Veterans Affairs should direct the Office of Asset Enterprise Management to clearly and completely document the selection process for all supportive-housing EULs from pre-development through completion of VA's development phase in keeping with internal control standards and VA policy.

    Comments: On September 19, 2017, an official from VA's Office of Capital Operations and Program Service said that the Office of Asset Enterprise Management (OAEM) is working with the Office of Information and Technology to ensure that the Enhanced Use Lease (EUL) Directive, Handbook, and Attachments are 508 compliant. This requirement is the last step in VA's internal process to certify and publish the updated EUL Directive and Handbook. Once OAEM receives an estimate when this process will be completed, VA will provide GAO a status update with the expected completion date for the updated EUL Directive and Handbook.
    Recommendation: To improve VA's supportive-housing EUL program and meet the needs of homeless veterans, the Secretary of Veterans Affairs should direct the Office of Asset Enterprise Management to update its EUL policy to (1) address the current authority for developing supportive-housing; and (2) specify how to identify properties for supportive-housing EULs to meet the needs of homeless veterans.

    Recommendation: To increase awareness of the health and legal consequences of FGM/C among visa recipients, the Secretary of State should update the Foreign Affairs Manual to require posts located in countries where FGM/C is commonly practiced to directly provide information about FGM/C to nonimmigrant visa recipients in the same manner as is done for immigrant visa recipients.

    Comments: The Department of State (State) concurred with this recommendation, and noted in a September 2016 update that it will expand the pool of visa applicants who will receive information on FGM/C. Specifically, once its transition to providing paperwork electronically during the visa application process is complete, State will directly distribute the fact sheet on FGM/C to both immigrant and nonimmigrant visa recipients. According to State, both immigrant and nonimmigrant visa recipients will have to certify that they have read and understood the fact sheet on FGM/C before signing and submitting their respective visa applications. As of August 2017, State has not yet completed these actions.

    Recommendation: In order to clarify the federal role related to freight-related local traffic congestion, in implementing MAP-21 and any subsequent reauthorization, in its final guidance on state freight plans, Secretary of Transportation should incorporate additional information to help states define and prioritize local community impacts of national freight movements, including traffic-congestion impacts, and to establish what data could be consistently collected and analyzed in order to prioritize impacts of freight on local traffic congestion.

    Comments: On October 14, 2016, DOT finalized guidance on state freight plans and state freight advisory committees. Among other things, the guidance incorporates information to help states define and prioritize local community impacts of national freight movements and identifies data sources to assist in collecting and analyzing data to prioritize impacts of freight on local traffic congestion. Including this information in the final guidance to states, will assist in better defining the extent of freight-related congestion, prioritizing projects to mitigate such impacts at both the state and national level and in DOT establishing a clear federal role for addressing these issues.
    Recommendation: In order to clarify the federal role related to freight-related local traffic congestion, in implementing MAP-21 and any subsequent reauthorization, the Secretary of Transportation should include in the National Freight Strategic Plan a written statement articulating the federal role in freight-related local congestion impacts, by clearly identifying potential objectives and goals (under the general area DOT has established for the Freight Transportation Conditions and Performance Report of reducing adverse environmental and community impacts) for mitigating local congestion caused by national freight movements and the type of role federal and state stakeholders could play in achieving each objective and goal, and including a written strategy for improving the availability of national data needed to quantify, assess, and establish measures on freight trends and impacts on local traffic congestion.

    Priority recommendation

    Comments: DOT concurred with this recommendation, and has developed a draft National Freight Strategic Plan, for which the public comment period ended in April 2016. In December 2016, DOT noted that it is continuing to review public comments received and beginning work on a final draft plan that will address the federal role in freight-related local congestion, as well as the other elements of GAO's recommendation. DOT stated that it plans to release a final National Freight Strategic Plan by its statutory due date of December 4, 2017. A number of our recommendations to DOT to address national freight issues are tied to the issuance of DOT's National Freight Strategic Plan.Implementing our recommendation in connection with development of the freight strategic plan expeditiously would help maximize the efficient movement of freight and ensure accountability around identified national interests and goals.

    Recommendation: The Secretary of the Treasury should issue guidance on how funding or assistance from other government programs can be combined with the NMTC including the extent to which other government funds can be used to leverage the NMTC by being included in the qualified equity investment.

    Comments: Although the Department of the Treasury (Treasury) has not issued guidance on how funding or assistance from other government programs can be combined with the NMTC, as GAO recommended in July 2014, it has taken steps toward addressing this action. Specifically, the Community Development Financial Institutions Fund (CDFI Fund), which administers the NMTC program, awarded a contract in September 2015 for new empirical research assessing the extent to which other government programs are being used to leverage the NMTC. CDFI officials have said that this research would help examine the various types of public support used for community development projects and assess the depth of the subsidy necessary to mitigate risk and attract new private capital to businesses located in low-income communities. As of October 2016, CDFI officials anticipate that the contract should be completed in March 2017.
    Recommendation: The Secretary of the Treasury should ensure that controls are in place to limit the risk of unnecessary duplication at the project level in funding or assistance from government programs and to limit above market rates of return, i.e., returns that are not commensurate with the NMTC investor's risk.

    Priority recommendation

    Comments: The Community Development Financial Institutions Fund (CDFI Fund), which administers the NMTC program, has developed a plan to issue guidance to help ensure that Community Development Entities (CDE) accurately report on sources of public funds and projected internal rates of return, as GAO recommended in July 2014. In January 2016, the CDFI Fund released updated guidance that explains in more detail how CDEs should report data on the use of other public sources in financing NMTC projects. This guidance should help ensure that CDEs accurately report on sources of public funds. As of October 2016, CDFI Fund officials are also evaluating changes to guidance on how CDEs are to report different project rates of return. The CDFI Fund awarded a contract in September 2015 for new empirical research assessing the extent to which other government programs are being used to leverage the NMTC. According to CDFI officials, this research will help them to examine the various types of public support used for community development projects and assess the depth of the subsidy necessary to mitigate risk and attract new private capital to low-income communities.
    Recommendation: The Secretary of the Treasury should ensure that the Community Development Financial Institutions (CDFI) Fund reviews the disclosure sheet that CDEs are required to provide to low-income community businesses to determine whether it contains data that could be useful for the Fund to retain.

    Status: Open

    Comments: The Department of the Treasury (Treasury) reported that as of September 2016, the CDFI Fund had reviewed the CDE disclosure sheets provided to low-income community businesses, as GAO recommended in July 2014, and determined that useful data from the sheets were already being collected through other data-gathering tools used by the Fund. In January 2016, CDFI officials reported that they did an initial comparison of the data on the disclosure sheets to the data in the Community Investment Impact System (CIIS)which is the system CDEs use to submit reports to CDFI. Officials said they were continuing to investigate any differences between the disclosure sheets and CIIS. Officials also said that they are performing additional analysis on any new data reporting requirements to meet the requirements of the Paperwork Reduction Act, which aims to minimize the burden that agency data collections impose on the public.

    Recommendation: As FEMA determines the scope of its efforts to revise its existing guidance, the Secretary of the Department of Homeland Security (DHS) should direct the Administrator of FEMA to update existing guidance to include additional information on and options for mitigating the risk of flood damage to agricultural structures to reflect recent farming developments and structural needs in vast and deep floodplains.

    Comments: To obtain information for updating existing guidance, FEMA engaged a contractor in April 2016 to conduct Phase 1 of a study evaluating recent farming developments. The June 2016 report from the contractor provided FEMA with information on the types of flood damage agricultural buildings and contents can sustain, required mitigation measures under NFIP, and insurance that is currently available to farmers. Phase 2 of the study is underway. This phase will identify the number and types of agricultural structures and the legislation, regulations, and various agency programs affecting the management of these structures; analyze the feasibility of mitigation options for these structures across different types of floodplains; and explore rating guidelines and potential mitigation techniques that could result in reduced risk or rates for agricultural structures. FEMA expects to receive a draft of the Phase 2 study from the contractor in July 2017. GAO will continue to monitor FEMA's progress.

    Recommendation: To establish full-risk rates for properties with previously subsidized rates that reflect their risk for flooding, the Secretary of the Department of Homeland Security (DHS) should direct the FEMA Administrator to develop and implement a plan, including a timeline, to obtain needed elevation information as soon as practicable.

    Comments: As we reported in February 2016 in GAO-16-190, FEMA has taken limited action to implement this recommendation. For example, FEMA noted that the agency would evaluate the appropriate approach for obtaining or requiring the submittal of information needed to determine full-risk rates for subsidized properties. FEMA also said it would explore technological advancements and engage with industry to determine the availability of technology, building information data, readily available elevation data, and current flood hazard data that could be used to implement the recommendation. However, FEMA officials also said that the agency faced a cost challenge with respect to elevation certificates and that obtaining these certificates could take considerable time and cost. They noted that requiring policyholders to incur the cost of obtaining elevation certificates would not be consistent with NFIP's policy objective to promote affordability. The officials added that the agency encourages subsidized policyholders who seek to ensure the appropriateness of their NFIP rates to voluntarily submit elevation documentation.

    Recommendation: To improve the ability of the Army and local communities to manage future base closures, the Secretary of Defense should direct the Secretary of the Army to issue, consistent with DOD guidance, guidance on specific levels of maintenance to be followed in the event of a base closure based on the probable reuse of the facilities.

    Comments: In DOD's comments on our report, it stated that it concurred with our recommendation and that the Army agrees to publish property maintenance guidance prior to closing installations in the event of future base closures. In June 2016, DOD stated that the Department is prohibited by Congressional/statutory language to plan for or implement future BRAC execution.
    Recommendation: To improve the ability of DOD and the local communities to respond to future growth actions, the Secretary of Defense should direct the Secretaries of the Army, the Navy, and the Air Force to consider developing a procedure for collecting service members' physical addresses while stationed at an installation, annually updating this information, and sharing aggregate information with community representatives relevant for local planning decisions, such as additional population per zip code, consistent with privacy and force protection concerns.

    Status: Open

    Comments: DOD partially concurred with this recommendation and stated that it agrees that information pertaining to the physical location of installation personnel helps affected communities plan for housing, schools, transportation and other off-post requirements in support of installations. It further stated that existing policy requires the military departments to provide planning information, including base personnel, to states and communities to support the establishment or expansion of a military base. In the event of future basing decisions affecting local communities, DOD stated that it will work with the military departments to assess and determine the best means to obtain, aggregate, and distribute this information to help ensure adequate planning information is made available. In June 2016, DOD stated that the Department is prohibited by Congressional/statutory language to plan for or implement future BRAC execution.
    Recommendation: The Secretary of Defense should direct the Secretaries of the Army and the Air Force to consider creating or designating a civilian position at the installation level to be the focal point and provide continuity for community interaction for future growth installations and to consider expanding this position to all installations. This position may be a collateral duty.

    Status: Open

    Comments: DOD partially agreed with this recommendation. It stated that it agrees with the need for a designated position at the installation level to work with the community and will ensure this requirement is being met through current practices and in accordance with each military department's personnel system. In many of the growth impacted communities, installation officials serve as ex-officio members of the community's growth management organization and relevant installation staff, including those engaged with public works, housing, education, and land use planning, and coordinate as needed with their civilian community counterparts. In July 2015, DOD stated that in the event the Department of Defense proceeds with future realignments that could result in a reduced footprint, there are provisions for Base Transition Coordinators (BTCs) to be designated as a liaison with the affected community. In the event these future realignments result in an expanded footprint or personnel growth, the Department would consider this recommendation at that time. In June 2016, DOD stated that the Department is prohibited by Congressional/statutory language to plan for or implement future BRAC execution.

    Recommendation: To help meet the requirements of the Recovery Act's Section 1602 provisions to help ensure the long-term viability of the buildings, the Secretary of the Treasury should assess the extent to which HFAs are utilizing information provided to them by project owners duringthe 15-year compliance period, taking into account the level of investor equity in projects.

On January 24, 2017, the agency informed us that they plan no further action on this recommendation.

    Recommendation: The Interagency Council and the Office of Management and Budget; in conjunction with the Secretaries of HHS, HUD, Labor, and VA should consider examining inefficiencies that may result from overlap and fragmentation in their programs for persons experiencing homelessness. As a starting point, the agencies could use the program information from this report to further analyze the degree and effects of overlap and fragmentation. The results of this assessment could be used to take actions to reduce any identified inefficiencies and therefore better leverage their resources. Actions may include streamlining services offered within specific programs or by agencies, identifying programs that could benefit from further research or evaluations, or consolidating programs or services to reduce administrative costs.

    Recommendation: To help reduce voucher program costs or better ensure the efficient use of voucher program funds, the HUD Secretary should provide information to Congress on housing agencies' estimated amount of excess subsidy reserves. In taking these steps, the Secretary should determine a level of subsidy reserves housing agencies should retain on an ongoing basis to effectively manage their voucher programs.

    Comments: According to HUD officials, the Department quarterly reports to Congress and the Inspector General (1) total housing agency reserves and (2) 8.5 percent of total housing agency reserves, the amount the Department has determined that housing agencies collectively should maintain on an ongoing basis to manage the voucher program (all other reserves are generally considered "excess"). HUD has also taken steps to determine an appropriate level of subsidy reserves that individual housing agencies should retain on an ongoing basis to effectively manage their voucher programs. Specifically, HUD has proposed that small housing agencies (fewer than 50 vouchers) should maintain 12 percent of their monthly renewal budget authority in reserves; medium housing agencies (50-249 vouchers) should maintain 6 percent of monthly renewal budget authority; and large housing agencies (250 or more vouchers) should maintain 4 percent of monthly renewal budget authority. In setting these reserve levels, HUD considered several factors including program size and budget and voucher utilization rates, among other things. This recommendation will remain open until HUD implements these proposed changes. As of September 2017, HUD officials said that the administration was determining whether or not to move forward with this proposal.

    Recommendation: To improve FTA's ability to determine the extent to which transit grants, current formula incentives, and FTA program activities have had a positive effect on local transit agency performance--and to the extent that FTA finds current data sources to be appropriate and reliable for such purposes-- the Secretary of Transportation should direct the FTA Administrator to further analyze and use data submitted to FTA by local transit agencies. As part of this analysis, FTA may want to identify and evaluate, when applicable, (1) the extent to which transit grant programs are accomplishing their established goals; (2) the areas of performance in which FTA should concentrate its program activities to increase the performance of local transit agencies and the federal transit program in general; and (3) the extent to which formula incentives and other performance accountability mechanisms have influenced the activities and performance of local transit agencies.

    Recommendation: In developing legislation for a national reporting system for the biological laboratory community, Congress may wish to consider provisions for the agency it designates as responsible for the system to take into account the following in design and implementation: (1) including stakeholders in setting system goals; (2) assessing labs' organizational culture to guide design and implementation decisions; (3) making reporting voluntary, with open-reporting formats that allow workers to report events in their own words and that can be submitted by all workers in a variety of modes (Web or postal), with the option to report to either an internal or external entity; (4) incorporating strong reporter protections, data deidentification measures, and other incentives for reporting; (5) developing feedback mechanisms and an industry-level entity for disseminating safety data and safety recommendations across the lab community; and (6) ensuring ongoing monitoring and evaluation of the safety reporting system and safety culture.
