    Recommendation: The Undersecretary for Health should take additional steps to ensure that VA's website reports health care quality measures that cover a broad range of health care services, highlights key differences in the clinical quality of care, and presents this information in an easily accessible and understandable way (Recommendation 1).

    Recommendation: The Director of OMB should submit or reference agencies' report modification proposals in the President's annual budget as required by GPRAMA.

    Comments: In August 2017, the Office of Management and Budget stated it would include a list of report modification proposals in the President's fiscal year (FY) 2019 budget as required by GPRAMA. We will close this recommendation when the FY 2019 President's budget is released.

    Recommendation: To strengthen MDA's acquisition efforts and help support oversight and to ensure MDA makes sound investment decisions on improving homeland ballistic missile defense, the Secretary of Defense should direct MDA to make the department's analysis of alternatives an integral part of its planning effort and delay any decisions to begin development of the new GMD Redesigned Kill Vehicle until: a) the department's analysis of alternatives is completed and identifies the best solution to pursue; and b) Congressional and DOD decision makers have been provided the results of that analysis.

    Comments: DOD partially concurred with our recommendation. As of July 2017, MDA has yet to finalize the analysis of alternatives (AOA) report. MDA officials stated the results of the analysis from the AOA were reached in early fiscal year 2015 and informed the decision to pursue the RKV. The AOA results were summarized and briefed to staff members from the congressional defense committees and senior DOD leadership. MDA subsequently submitted the report to DOD's Office of Cost Assessment and Program Evaluation (CAPE) for a sufficiency review but an official from the Office of the Secretary of Defense stated in July 2017 that CAPE has yet to perform the review. We await the final AOA report to make a determination of whether the recommendation was implemented.
    Recommendation: To strengthen MDA's acquisition efforts and help support oversight, drawing from information it already has, the Secretary of Defense should direct MDA to report annually to Congress its plans for, and achieved progress in developing and delivering integrated Ballistic Missile Defense System (BMDS)-level capabilities. This reporting should include: a) planned integrated BMDS-level capabilities, including dates for when capability is planned for delivery; and b) element-level upgrades needed for delivery of the integrated BMDS capability, including dates that these upgrades need to be available for integration into the BMDS capability.

    Comments: In providing comments on this report, the Department of Defense concurred with our recommendation to annually report to Congress its plans and progress made in delivering integrated BMDS-level capabilities, including delivery dates for the capabilities and element upgrades needed to support these capacities. Previously, MDA planned to provide Congress a table summarizing the development and delivery of integrated BMDS capabilities annually, beginning in the 2016 BMDS Accountability Report (BAR). We reviewed the 2016 BAR and determined the report did not include the full extent of information regarding plans and progress made in delivery integrating BMDS-level capabilities described in our recommendation. To remain consistent with the methodology for our annual missile defense review, we will assess the 2017 BAR in next year's review to determine if it includes this information.

    Recommendation: To help ensure the accuracy of its Senior Federal Official Travel Reports, the Administrator of GSA should collect additional information from agencies on instances where travel is not being reported because of an exemption for intelligence agencies, as opposed to some other reason, and include such information in its reports where departmental data do not include trips pursuant to an agency's exercise of a reporting exemption.

    Recommendation: To help ensure that agencies are tracking the effect of strategic sourcing on small businesses, OMB's Administrator for Federal Procurement Policys should monitor agencies' compliance with the requirement to maintain baseline data and performance measures on small business participation in strategic sourcing initiatives.

    Comments: NASA officials provided an update in March 2017. They stated that they consider the inclusion of small businesses and small disadvantaged businesses throughout the strategic sourcing process and that they track performance on their small business goals at the agency and buying office level. However, they stated they had no plans to track baseline data and performance measures on small business inclusion for individual strategic sourcing efforts or strategic sourcing efforts grouped by categories because they did not see the benefit of doing so. As we stated in our report, OMB memorandums require baseline data and a measure of the change in small business spending for each individual initiative. Therefore, we continue to believe this recommendation has merit and should be fully implemented.

    Recommendation: To improve future real property cost-savings initiatives and promote reliability and transparency, the Director of OMB, in collaboration with Federal Real Property Council agencies, develop clear and specific standards for: (1) identifying and reporting savings that help ensure common understanding of the various types of cost savings; (2) consistently reporting across categories and agencies; and (3) sufficiently documenting, validating, and reviewing results.

    Comments: In March 2015, GAO met with OMB officials about their implementation of a new cost estimating model that would largely implement this recommendation. OMB officials said they had engaged a contractor to develop the model and in the coming months they would be contacting member agencies on the Federal Real Property Council to obtain their feedback, with hopes of eventually incorporating the model into ongoing real property reform efforts. As of May 2016, GAO was inquiring about the status of this effort and was anticipating a meeting with OMB in the coming weeks to discuss this and other related real property efforts aimed at addressing GAO's high risk designation for this area.

    Recommendation: FSOC and OFR should clarify responsibility for implementing requirements to monitor threats to financial stability across FSOC and OFR, including FSOC members and member agencies, to better ensure that the monitoring and analysis of the financial system are comprehensive and not unnecessarily duplicative.

    Priority recommendation

    Comments: As of October 2016, FSOC staff have said FSOC and its members, including OFR, understand their responsibilities, saying that meetings of FSOC's Systemic Risk Committee help to ensure that FSOC member agencies have clarity on their responsibilities and noted that the committee operated under a charter. However, our review of the charter found that it does not clarify responsibilities for monitoring threats to financial stability. They also stated that actions OFR and the Federal Reserve (both of which serve on the Systemic Risk Committee) agreed to take in response to a recommendation in a GAO report issued in February 2016 would help to clarify these responsibilities. However, these represent just two of FSOC's member agencies; similar collaborative steps by other agencies would support the clarity of roles for monitoring threats to financial stability. We maintain that more specific action from FSOC and OFR, including FSOC member and member agencies, is needed to address this recommendation that ensures clarity of roles and responsibilities in proactively and comprehensively monitoring for potential emerging threats in the financial system. Our past work has shown that the lack of clear roles and coordination can lead to duplication, confusion, and regulatory gaps.
    Recommendation: FSOC and OFR should clarify responsibility for implementing requirements to monitor threats to financial stability across FSOC and OFR, including FSOC members and member agencies, to better ensure that the monitoring and analysis of the financial system are comprehensive and not unnecessarily duplicative.

    Comments: As of March 2017, OFR has taken some steps to work with the Board of Governors of the Federal Reserve System, a member agency of FSOC, to organize semi-annual meetings to jointly discuss views from their respective monitoring of the financial system for risks. We continue to monitor FSOC and OFR actions that would be responsive to clarifying responsibilities for monitoring threats to financial stability across all the agencies that are members of FSOC.
    Recommendation: To strengthen accountability and collaboration in FSOC's decision making, FSOC should establish a collaborative and comprehensive framework for assessing the impact of its decisions for designating FMUs and nonbank financial companies on the wider economy and those entities. This framework should include assessing the effects of subjecting designated FMUs and nonbank financial companies to new regulatory standards, requirements, and restrictions; establishing a baseline from which to measure the effects; and documenting the approach.

    Comments: In response to an April 2017 presidential memorandum, Treasury is conducting a review of FSOC's designation process, including an assessment of the effects of designating FMUs and nonbank financial companies. This review will result in a report. We will update the status of this recommendation after we have reviewed the report.
    Recommendation: To strengthen accountability and collaboration in FSOC's decision making, FSOC should develop more systematic forward-looking approaches for reporting on potential emerging threats to financial stability in annual reports. Such an approach should provide methodological insight into why certain threats to financial stability are included or excluded over time, separate current or past threats from those that are potentially emerging, and prioritize the latter.

    Recommendation: To help ensure that Congress is fully apprised of efforts to resolve a case, Congress may wish to consider amending USERRA to require DOJ and OSC to report on additional time taken to resolve a matter after they decline representation.

    Recommendation: To help ensure that that servicemembers who file complaints are adequately being informed of their USERRA complaint process rights in accordance with VBIA 2008, Congress may wish to consider amending USERRA to require DOL to report on the extent to which it is notifying complainants of their USERRA complaint process rights within 5 days of filing a complaint.

    Recommendation: To help ensure that DOJ handles state cases as expediently as private employer cases, Congress may wish to consider amending USERRA to specifically require DOJ to adhere to the same 60-day deadline for state employer matters that they must adhere to for matters against private employers.
