V. UNITED STATES, CT.CL.NO. 33-64.

UPON RECEIPT OF YOUR CLAIM IN THE FORM INDICATED ABOVE YOUR ENTITLEMENT TO AN ADDITIONAL PAYMENT WILL BE CONSIDERED ON THE BASIS OF THE FACTS YOU PRESENT AND ON THE BASIS OF ANY ADDITIONAL FACTS AND INFORMATION PROVIDED BY THE DEPARTMENT OF THE NAVY.
